Notes The numbering of the fields corresponds to the labels on the form image above. You can include LaTeX font and size information in the fields. Text fields can be left blank. If you set the Form Parameter to 'None', the barcode area will be omitted, rather than just left blank. The barcode is limited to 6 characters. Available characters are the 26 letters, 10 digits, space and $%.+-/. Letter case is ignored. Form layout based on Roger Eyland's original design.
